Ho-Chunk Hope is a Diabetes Prevention Program (DPP) that is based on a nationally recognized curriculum developed from the Center for Disease Control. The Whirling Thunder Diabetes Program offers a 26-session version of DPP that lasts for 12 months. We are currently in pending status for being a nationally certified program.

 

DPP addresses many health-related habits and areas. The most prominent topics covered are:

  • Nutrition
  • Exercise
  • Self-Monitoring
  • Social Support
  • Weight Loss
  • Problem Solving
  • Behavior Change
  • Stress and Stimulus Control
  • Goal Setting

Participants will receive consistent support from staff members and learn how to set (SMART) goals that they will work toward achieving during the program.

 


 

DPP goals include measuring weight and activity minutes for every session. At the end of the 26 sessions, a weight loss goal of 5% body weight is hoped to be achieved.

Who should join?

 

If you have these risk factors, you may be at higher risk than others for prediabetes and type 2 diabetes.

  • You are overweight.
  • You are 45 years of age or older.
  • Your parent or sibling has type 2 diabetes.
  • You are physically active fewer than 3 times per week.
  • You ever gave birth to a baby that weighed more than 9 pounds.
  • You ever had diabetes while pregnant (gestational diabetes).

Race and ethnicity also affect your risk. African Americans, Hispanic/Latino Americans, American Indians, Pacific Islanders, and some Asian Americans are at particularly high risk for type 2 diabetes.
