PURPOSE STATEMENT

The purpose of the Endocrinology Clinical Services component of the Whirling Thunder Diabetes Program is to provide high quality specialty care for persons with diabetes and other disorders of the endocrine system.

POLICY STATEMENTS

It is the administrative policy of the Endocrinology Clinical Services program to provide direct diabetes specialty care that is consistent with the adopted Indian Health Standards of Care for Adults with Type 2 Diabetes. Diagnostic criteria, treatment practices, self-care management strategies, patient education principles and overall risk-reduction of co-morbid conditions are delivered according to adopted Indian Health Service Diabetes Standard of Care.

Delivery of endocrinology services is provided by licensed medical professionals. Coordination of endocrinology clinical services is interfaced seamlessly with the primary care providers of the Indian Health Service for continuity.

PROCEDURE OF OPERATION

The general procedure of endocrinology clinical services program is to provide a monthly specialty endocrine clinic based from the Winnebago Tribal Health Department offices within the Indian Health Services Hospital.

DESCRIPTION OF SERVICES PROVIDED

Patient intake, summary of diabetes patient care is reviewed with each patient. Measurements and screenings include vital signs, height and weight, finger stick glucose, body compostion and depression screening.

Diabetes patient education includes; meter downloading and review, meter set-up and demonstration, nutrition education, fitness/ exercise education, insulin administration teaching and food care education.

Examinations provided include a complete review of systems based on patient presentation and also annual diabetes foot examinations. Minor nail and callus care is available. Medications and laboratory orders are processed. Immunizations are administered according to the Standards of Care. All documentation is completed into the Electronic Health Record. Referral care is coordinatoed and follow-up care is scheduled. Patient evaluations for input into the impovement of the endocrinology clinic are collected at the check-out point.
